The Laurel Health Centers are on a mission to improve access to care across rural northcentral PA. This spring, we expanded our primary care and dental services to Sullivan County, Pennsylvania.

Laurel Health worked quickly and diligently in collaboration with local leadership to preserve an important safety-net health resource as former tenants retired or withdrew operations from the county. The former Sullivan County Medical Center and Sullivan County Dental Clinic transitioned to Laurel Health’s ownership to become one unified health and dental center named the Sullivan County Laurel Health Center, located at 217 King Street in Laporte, PA.

We are excited to welcome family medicine physician Samuel Schrack, DO, and his team to Laurel Health as part of this transition. He brings over 50 years of wide-ranging healthcare expertise to the organization and sees patients of all ages.

His military experience sparked the path to a healthcare career where he found his lifelong calling to practice medicine as a primary care doctor in rural communities. After graduating from the US Army Medical Training Center as an honors graduate while training as a battlefield medic during the Vietnam era, he was employed as a licensed practical nurse in Lock Haven Hospital’s operating room. He continued advancing his medical education to become a physician assistant and then a family medicine physician.

Dr. Schrack is passionate about ensuring rural patients receive the care and attention they need close to home. “I enjoy everything about family medicine—all the different aspects of care and wellness—but what I find most rewarding are the patient relationships and successful outcomes,” he shares.

His clinical background comprises both inpatient and outpatient care settings, including primary care, occupational health, emergency medicine, ambulatory care, and medical military service. Dr. Schrack also served in the Pennsylvania Army and Air National Guard, and he is a Retired Major with the USAF Medical Corps, where he was awarded the Meritorious Service Medal.

Across his storied career, Dr. Schrack has provided medical care at hospital facilities, clinical institutions, and outpatient practices in Lock Haven, Mill Hall, Williamsport, and Laporte, PA. He has delivered over 475 infants. He has also served as a clinical assistant professor at Lock Haven University and Hershey S. Medical Center’s College of Medicine, Chairman of Legal & Governmental Affairs for the Pennsylvania Academy of Family Physicians, and President of the Lycoming County Medical Society.

In his free time, he enjoys attending Penn State sports events, gardening, remodeling, music, and spending time with his family.
